Abstract

CV. Tirta Indo Megah merupakan perusahaan yang bergerak di bidang produksi Air Minum Dalam Kemasan (AMDK). Berdasarkan data produksi kemasan cup 150 ml pada periode Mei–Oktober 2024, ditemukan persentase cacat yang melebihi standar perusahaan yaitu 0,2%, dengan angka tertinggi mencapai 1,11%. Jenis cacat produksi meliputi lid miring, bocor, kurang isi, dan cup pecah. Penelitian ini bertujuan untuk mengidentifikasi dan menganalisis penyebab kegagalan potensial dalam proses produksi AMDK cup 150 ml serta memberikan rekomendasi perbaikan. Metode yang digunakan adalah Fuzzy Analytical Hierarchy Process (Fuzzy AHP) untuk memberikan bobot kriteria penilaian (Severity, Occurrence, Detection, dan Expected Cost), yang selanjutnya diintegrasikan dalam Multi Attribute Failure Mode Analysis (MAFMA) untuk menentukan prioritas penyebab kegagalan. Hasil penelitian menunjukkan bahwa faktor paling berpengaruh adalah "material cup tidak sesuai standart" dengan bobot tertinggi. Rekomendasi yang diberikan meliputi pengawasan kualitas bahan baku cup. Penelitian ini diharapkan menjadi acuan dalam upaya perbaikan mutu dan efisiensi proses produksi perusahaan.
