Abstract

PT. Malindo Intitama Raya is a plastic furniture manufacturing company that relies on injection molding machines operating continuously 24 hours a day. The high frequency of unplanned downtime compared to planned downtime, along with fluctuations in the number of defective (reject) products on the main machine — a Haitian HTF780X — has the potential to hinder the smoothness of the production process and reduce machine effectiveness. This study aims to measure the machine's effectiveness level using the Overall Equipment Effectiveness (OEE) method, identify sources of loss through Six Big Losses, analyze potential failures using Failure Mode and Effect Analysis (FMEA), and formulate improvement proposals using a fishbone diagram. Based on historical data processing for the period January to December 2025, the average OEE value of the Haitian HTF780X machine was obtained at 39.46%. This value is still far below the World Class OEE standard of 85%. The low machine effectiveness is influenced by the average values of Availability (73.28%), Performance Rate (62.64%), and Quality Rate (85.91%). The dominant losses stem from Reduce Speed Loss and Equipment Failure/Breakdown Loss. From the FMEA analysis of 20 identified failure modes, 4 priority failures with the highest Risk Priority Number (RPN) values were obtained, namely: material backflow, non-optimal material oven process, ejector jamming, and leaking sleeder. Root cause analysis using a fishbone diagram shows that these failures were caused by component wear (nozzle, check ring, ejector pin, and sleeder), a preventive maintenance system based on machine operating hours that has not yet been optimized, inappropriate injection parameter settings, and high raw material moisture content. The recommended improvement proposals include implementing a structured preventive maintenance schedule, standardizing injection parameters and calibrating temperature sensors, developing a daily/per-shift routine inspection checklist, and conducting technical training for machine operators.
