Abstract

ABSTRACT Mokhamad Lutfi Suryo Raharjo, Master of Industrial Engineering Study Programme, Postgraduate Programme, Nationel Institute of Technology Malang, February 2026, Analysis of The Implementation of The Hospital Occupational Health and Safety Management Systems (SMK3RS) in East Java (A Case Study of Regional General Hospitals), Thesis Supervisor: (1) Ir. Fourry Handoko, ST.,SS.,MT.,PhD.,IPU, (2) Dr. Renny Septiari, ST., MT. Occupational safety and health (K3) is a fundamental aspect in modern industry and organizations, which is related to maintaining employees in safe and healthy working conditions, thus impacting productivity in the workplace. Hospitals are part of industrial organizations that have a high level of risk of potential occupational safety and health hazards. The Indonesian government through its regulations requires every hospital to implement the Hospital Occupational Safety and Health Management System (SMK3RS) with the aim of creating optimal occupational safety and health. Karsa Husada Regional General Hospital has implemented SMK3RS but there is still a fairly high level of work accidents, this encourages further research related to the implementation of SMK3RS. This study aims to identify potential risks or hazards, conduct risk level assessments, and obtain risk control responses that can be applied to 3 hospital installation units that are the focus of this research object and become a priority program for hospital management in handling risks or hazards. The use of the Job Safety Analysis (JSA) method helps identify hazards at each stage of the work process flow, the Hazard Identification Risk Assessment and Risk Control (HIRARC) method helps in assessing risks and their control, and the Failure Mode and Effect Analysis (FMEA) method is used to determine the priority of risk handling with the parameter of the Risk Priority Number value which is at the threshold of the priority handling. The results of processing statistical data on work accidents in 3 installation units show that the Frequency Rate (FR) is 1,131 lost work hours per one million work hours, the Severity Rate (SR) is 141 lost work days, and the Incident Rate (IR) is 27.14%. In the analysis of potential hazard risk identification, 21 potential hazards were found spread across the three installation units, of which 8 potential hazards were at a high level (High Risk), namely in the CSSD and Laundry installation units, the risk level with a high risk RPN of 324 and 240, in the IPSRS installation unit, the risk level with a high risk RPN of 280 and 224, while in the Laboratory and BDRS installation units, the risk level with a high risk RPN of 315, 280, 280, and 270. Risk control strategies are implemented by applying the principles of the Hierarchy of Control (HoC) including elimination, substitution, engineering controls, administrative controls, and the use of Personal Protective Equipment (PPE) in accordance with the minimum standards set by SNI and the Ministry of Health.
